Fill Out the Return to Campus Form

Dear LSU Community,
The fall semester is rapidly approaching, which means it’s vital that we begin the first steps of our TIGER Check COVID-19 Monitoring System, as outlined in our Roadmap to Fall, to prepare our community for a successful first day of class and beyond.
Foremost, the university is asking each person on campus to complete the Return to Campus Form as soon as possible. This one-time questionnaire will ask you to provide contact, demographic, and LSU-affiliated information. Additionally, you will be asked to think about and provide information regarding health-related plans should you begin to experience any COVID-19 related symptoms or come into close contact with someone who has tested positive for the virus. This information will help LSU prepare and plan for a safe return to campus and provide you with assistance, should you need it.
You will receive an email soon with more information about other components of the TIGER Check COVID-19 Monitoring System. In brief, the LSU community will be required to track their symptoms daily, isolate if they feel ill, get tested if applicable, exercise caution daily, and report positive tests and close contacts.
Everyone must participate in these steps to enable the efficient identification and swift containment of potential COVID-19 outbreaks. We urge you to visit lsu.edu/roadmap/monitoring/index.php and to familiarize yourself with each step of the TIGER Check COVID-19 Monitoring System, so that you can make responsible and empowered decisions this semester.
Finally, we’re asking you to visit lsu.edu/roadmap/pledge and join us in taking the pledge to help protect the LSU campus community and keep fellow Tigers in good health during these unfamiliar circumstances. The safe resumption of operations at LSU and of our lives in general requires each member of the campus community to do their part.
