CHSE students wishing to make adjustments to their declared program or submit an academic appeal can do so through the Office of Student Services. Completed forms must be submitted to the links listed below.
If a student has a question regarding completing a form, please contact the Office of Student Services email: firstname.lastname@example.org.
These applications will be available between final exam week at the end of one semester and the last day to add classes in the next semester. Applications must be submitted to the Office of Student Services email. After the last day to add classes has passed, these applications will not be accepted again until the next final exam period begins.
